Federal Information System Attestation
I acknowledge that usage of data sourced from a federal agency through a federal information system may be monitored, recorded, and subject to audit. I also acknowledge that unauthorized use of the federal information system is prohibited and subject to criminal and civil penalties, and that use of the information system indicates consent to monitoring and recording.
I will comply with the Duke Health Confidentiality Agreement and Duke Health security and privacy policies. I will use appropriate safeguards to protect data from misuse and inappropriate use or disclosure.
